General Instructions:  The purpose of this form is to assist departments in the collection of data to develop an offender profile of persons revoked from community supervision in a county during fiscal year 2000.  Include revocations from deferred adjudication, regular supervision, and special programs.  This form may be used to define target populations for potential new or enhanced programs, services and facilities.  Please report the requested data for each specific county within your jurisdiction.  Please give actual counts, not percentages.  Make sure that all blanks are filled, even if they are zero.

Reporting Instructions: Counties whose community supervision population size is 50 or less are required to report data on 100% of the population. Counties with revoke populations of 51 or more may collect the information using a sample of the population as outlined in the attached instructions. Sample sizes must be determined separately for both felons and misdemeanants. Information must be reported under the appropriate offense type: felony or misdemeanor.

If an offender has multiple offenses, count either the most serious or the offense with the longest sentence. Select the most appropriate category for the offense charged.
